Your credential record, on a dated document

For a credential holder who needs their own record as a dated file — an application, a credentialing packet, a travel assignment. USD 19.

What it contains

• Your credential read live from the Department of Health open data on the day you order — not from this site's last collection: number, type, status, first and last issue, expiry, continuing-education due date, and the Department's action flag exactly as published.
• Every other credential recorded under your full name — an RN who is also an ARNP, a pharmacy technician who became a pharmacist — on the same page.
• Your record in the federal NPI Registry, checked against your Washington credential: does your NPI list this license number? Credentialing forms ask for both, and they should agree. On a sample of physicians, some NPI records listed only a license from another state — the report says so when it is the case.
• What changed since this site's last collection: a renewal, a new expiry date, a new continuing-education deadline.
• Your renewal calendar: days left before expiry and before the continuing-education deadline.
• The source, the date, and the link to the Department's own search, so anyone you send it to can check each line.

Who it is for — and who it is not for

Only the credential holder, about their own credentials. It is not a background check and not a report on someone else: we do not sell a report about a person to an employer, an agency or anyone else. An employer verifying a credential should use the Department's own search or verification service.

Not a consumer reporting agency. This site and the datasets sold from it are not consumer reports under the Fair Credit Reporting Act, and must not be used to decide anyone's eligibility for employment, credit, insurance, housing, or any other purpose covered by the FCRA. If you need a background check, use a screening company that operates as a consumer reporting agency.

Price and how it works

USD 19 per report. No payment is taken on this site: you order below, we send the PDF within 24 hours, and a payment link follows. If the Department holds nothing for the number you gave, we say so and there is nothing to pay.
